There’s a pattern emerging in enterprise AI that challenges the usual tech narrative: the organizations moving fastest aren’t the ones rushing systems into production. If anything, they appear more controlled, more deliberate, and more structured than their peers. And yet, they’re the ones deploying faster. This is the governance trap. Most organizations assume governance is friction—something that slows innovation and gets in the way of progress. In reality, deferring governance is what creates the slowdown. The ones still stuck in pilot purgatory are, in many cases, stuck there because they’re thinking about governance backwards. Why the old approach worked Traditional IT governance was reactive by design. You built the system, deployed it, and locked it down. Access controls, audit trails, compliance reviews—these came after the thing existed, because you needed something to govern before you could govern it. This worked because traditional systems told you when they failed. Wrong permissions threw an access denied error. A broken integration surfaced an exception. A misconfigured rule produced a predictable, traceable error state. The system was deterministic: the same input produced the same output, every time. Governance could be reactive because failures were visible, discrete, and fixable. You saw the problem, you wrote the rule, you moved on. That entire model rests on an assumption that AI breaks completely. The invisible failure problem AI systems are probabilistic . The same input doesn’t always produce the same output. An agent that performed correctly in testing can produce a harmful or inaccurate output once it’s in production under conditions it hasn’t encountered before. And unlike traditional software, it won’t tell you. It will produce a confident, fluent, plausible-sounding answer—and that answer may be wrong in ways that don’t surface until users are complaining, downstream systems are corrupted, or an audit surfaces something nobody noticed was happening. This is the first reason reactive governance fails in AI: you can’t see all the failures in the demo. The demo worked on curated data, controlled conditions, the specific scenarios you thought to test. Production introduces the full complexity of the real environment—and the failures it exposes are the ones you didn’t anticipate, which are precisely the ones your post-hoc rules didn’t cover. The traditional response to this is to write more rules. Cover more edge cases. Add guardrails for every scenario the demo surfaces. But the edge case space in a probabilistic system is effectively infinite. You cannot patch your way to safety. For every edge case you’ve seen, there are a hundred you haven’t, and a governance model built from observed failures will always be one production incident behind. You’re not testing what you’re shipping When you build a governance layer after the fact and attach it to a deployed agent, you have changed what you’re running. The system that passed the demo wasn’t governed. The system you’re putting into production is. Those are not the same system. You didn’t test what you shipped. This matters because autonomous agents are, by definition, acting independently. The value proposition is that they execute without requiring human intervention at every step. But that value proposition only holds if you can trust that the agent will behave as expected across the full range of situations it will encounter—including the ones that weren’t in the demo. Bolt-on governance doesn’t give you that trust. It gives you a modified system with guardrails retrofitted onto behavior you validated without them. I can't tell if it's our data, our vendors, our architecture, or something else entirely.” If that sounds familiar, you're not alone and it says something about the state of enterprise AI adoption today. MIT's 2025 "GenAI Divide" study found that 60% of organizations evaluated custom or vendor-built enterprise AI systems, but only 20% reached the pilot stage, and just 5% made it to production. AI may show real promise in demos. But when it gets deployed and hits real data, real systems, and real approval chains, it stalls in pilot purgatory . It’s natural to blame the model, but here’s why that’s a mistake. The 80/20 rule in Enterprise AI LLMs and SLMs, while marvels of engineering, can be frustrating when they don't behave the way you want. So you look for a better one, maybe try to fine-tune it, or simply wait for the next upgrade. This approach rarely gets a stalled AI project off the ground, because the model is only 20% of the equation. Think of the model as a lightbulb. It can reason, summarize, and draft convincingly. But a lightbulb by itself doesn't deliver light. That takes electrical wiring to carry the power. The AI model needs that same kind of wiring or infrastructure. It has to connect to the systems that hold your data, know who's allowed to approve what, and reconcile conflicting definitions of things like "customer" or "closed deal" across every system that touches them. This is the crux of most enterprise AI challenges today. Businesses have spent heavily on the 20% (the model) and left the 80% (the infrastructure) mostly untouched. Paying an invoice means confirming the vendor is approved and the PO behind it is legitimate, the exact data the procurement agent already governs. That data is scattered across so many systems and formats that reconciling it takes judgment, and the finance team makes that judgment on its own instead of reusing procurement's. Ask the procurement AI agent if a vendor is approved and you get one answer. Ask the invoice AI agent and you get another. Now you've got two versions of the truth contradicting each other as "approved vendor" doesn’t mean the same thing everywhere. Some call this the agent trap . Every production AI system needs the same three ingredients, pulled from what already exists rather than rebuilt inside each new project. Knowledge. One shared, reconciled understanding of what your data actually means, so "vendor approved" means the same thing wherever it shows up. Governance. Permissions, policy, and compliance are enforced consistently, so every agent automatically knows what it's allowed to touch instead of that logic getting reinvented for every project. Actionability. Smarter tools, the same bottlenecks To push past that wall, many organizations add AI into the mix. Excerpt from our conversation with Bethany Warburton, PMP , Sr. Project Manager , Office of Information Technology at Brown University. "Don't Pave the Cow Path" — don't use AI to automate a broken, wasteful workflow. Take the time to re-engineer processes and do it right. What operational challenges were you trying to solve when you set out to modernize your enterprise AI foundation? As we look to meet budget constraints, our teams are being asked to do more with fewer resources. Introducing AI into our operations to help lift some of the effort from our team members makes so much sense, especially in areas where there is a lot of repetitive manual effort with data and rules/logic we can follow. For us, that started with contract management and procurement operations. The team processes thousands of contracts and requisitions each fiscal year that require time-consuming validation checks across multiple stages of the contract lifecycle. The processes were rule-based and repetitive, which made them strong candidates for intelligent automation. The results have been a significant year-over-year reduction in queue time which moves contracts through the end-to-end process faster and with greater accuracy. We also just went live with an AI solution for our research administration team for Funding Opportunity Announcements (FOA) where the technology is now parsing PDFs, cross-referencing checklists, allowing AI to extract structured data from FOA documents against a configurable checklist with citation references. The result has been to drop the review time from about 1 hour to 5–10 minutes per notice. Overall, we are still in the very early stages and are monitoring everything closely to make sure our teams can really maximize the value. We expect to expand and will move with as much speed as makes sense for our departments. Where has AI become operational across the organization, and where is it showing up most visibly? Overall, we don’t expect any near-term deployments where our stakeholders internally will have high visibility into an AI specific solution. Rather, we hope it's pretty seamless to them and they just notice more efficiency and accuracy. For example, removing the bottlenecks from the data validation checks in the contracting processes has reduced queue times. Stakeholders aren’t interacting with the solution, but they are benefiting from it. What has changed inside the organization since you began building this enterprise AI foundation? What’s changed most is the awareness of the need to change. However, I think it's less because of the platform and more because of the realities of operating in the world right now. We have to work more efficiently and effectively and departments are becoming aware that there are tools that can help them. We are looking to apply the lessons we’ve learned from these first few implementations, especially related to setting expectations with departments where readiness matters as much as technology. For example, getting the right stakeholders aligned, understanding the workflow before we look to automate it, and building trust with the teams who will use these tools is what best determines whether AI delivers value or just adds complexity. This groundwork makes everything else possible. How do you think about the operating model of an AI‑native enterprise—and is that where you’re steering the organization? We are exploring all mechanisms right now. I don’t think we’ll be early adopters of moving to a fully AI-native enterprise. But what we are focused on is making sure the foundation is right. That means starting with use cases where the rules are clear, the processes are well-defined, and the value is measurable. The truth is that successful AI deployment is not a question of model quality—or even funding. It is determined by approach. The 5% of organizations running AI at production scale aren't better resourced or more technically sophisticated than the 95% still stuck in pilot purgatory. They simply made different decisions. If no such delta can be named—and measured—the workflow is not a candidate for AI, no matter how elegant the implementation might be. If you can't point to the specific decision that drives an outcome, you're not designing a workflow—you're describing activity. Once an outcome is clear, the next step is to trace backward to the decisions that control it. Every meaningful business outcome is downstream of a small number of decisions made repeatedly over time: which cases to prioritize, which exceptions to escalate, which actions to take now versus later. These decisions are rarely slow because humans are incapable; they are slow because humans are overloaded. They require context scattered across systems, judgment applied under time pressure, and consistency across volumes that do not respect headcount. What high-value AI workflows actually look like This is where high-value AI workflows begin to reveal themselves. Access is not context When a customer is recorded in your CRM as “Acme Corp,” in your ERP as “Acme Corporation,” and in your contract system as “Acme Corporation, Inc.,” a human resolves that without thinking. Institutional knowledge, pattern recognition, a quick call to someone who knows. AI does not have that intuition. Without a shared semantic layer telling it that these three records represent the same entity, the AI is reasoning over three different things—and producing three different versions of the truth. This is not an edge case. It’s the normal state of enterprise data. Different systems were built at different times, by different teams, for different purposes. They were never designed to talk to each other. And until now, they didn’t have to—humans were doing the translation. When AI enters the picture, it inherits that fragmentation. And it cannot compensate the way humans do. Three kinds of data, three different problems The enterprise data landscape breaks into three categories, each of which creates a distinct failure mode for AI. Structured data —transactions, records, fields—is the most legible to machines. But legible isn’t the same as meaningful . A database can store a customer’s payment history without containing any information about what that history implies: whether a pattern of late payments reflects a cash flow issue or a billing dispute, whether a renewal at a lower rate was a concession or a strategic win. Unstructured data —documents, contracts, emails, policy files, meeting notes—is where most enterprise knowledge actually lives. Humans extract meaning from it constantly, through reading, judgment, and accumulated experience. AI systems have to reconstruct that interpretation on every query, from scratch, without the shared understanding that makes human interpretation fast and consistent. Operational state —what is actually happening across active workflows right now—is the category almost no enterprise captures in a form AI can reason over. Which deals are in flight. Which approvals are pending and why. Which exceptions were made last quarter and what precedent they set. Without it, AI agents are reasoning about the business as it appears in documentation, not as it operates in practice. Each of these gaps has consequences. Together, they explain why AI systems that perform well in controlled conditions degrade the moment they’re exposed to real enterprise complexity. Hallucination is a data problem The word “hallucination” implies a model defect—something going wrong inside the AI. In most enterprise deployments, that framing is misleading. The model is doing exactly what it was designed to do: generating a plausible, coherent response based on available context. The problem is that the available context is incomplete. It may sound contradictory, but CIOs are struggling to show enterprise AI ROI because it’s becoming less expensive to use and more expensive to operate. Model providers keep lowering per-token prices while total AI spending rises. As enterprises move from simple assistants to multi-step workflows, longer context windows, tool calls, retrieval, retries, and autonomous agent loops, boards begin asking: If AI is getting cheaper, why is our bill going up? The answer is that enterprise AI cost reduction is not primarily a procurement exercise. Negotiating a lower token rate or switching models may reduce one line item, but it does not correct the architecture that determines how many tokens, integrations, workflows, approvals, and engineering hours each task consumes. The real savings come from designing AI systems that use the minimum necessary intelligence, reuse enterprise context and workflow components, and keep humans involved only where judgment is required. Why cheaper AI models are not producing cheaper AI programs Despite the rising need for enterprise AI cost reduction, the cost of model inference has fallen dramatically. The blended cost of AI dropped by roughly two-thirds between early 2025 and early 2026, with a decline of more than 99% in the inference cost required to achieve GPT-3.5-level capability over approximately two years. At the same time, enterprise LLM spending increased sharply. Median inference spending rose several times over in 2025, while many enterprises exceeded their original AI budgets. The reason is straightforward: unit cost is falling, while consumption volume and operational overhead are expanding. A basic chatbot may receive a prompt, retrieve a small amount of information, and return an answer. An agentic workflow may plan a task, call multiple tools, retrieve data from several systems, evaluate intermediate results, retry failed steps, request approval, and update a system of record. While those interactions look similar to the user, they are not economically similar. It’s estimated that agentic workflows can consume five to 30 times more tokens than conventional chatbot interactions. Response refinement and iterative agent loops can also become the largest component of runtime cost. Each turn may resend accumulated instructions, retrieved content, tool outputs, and conversation history. As sessions grow, cost compounds. This is why focusing only on per-token pricing is misleading. A cheaper model executing an inefficient workflow can cost more than an expensive model used selectively. Where enterprise AI cost reduction does work Route each task to the right model Many enterprise tasks are narrow and repetitive, like classifying an invoice or extracting fields from a contract. These tasks do not require the most expensive model for every step. Instead, model routing assigns work according to complexity. A smaller or specialized model handles routine steps, while a more capable model is used only when the task requires deeper reasoning. This form of AI token cost optimization reduces spend without weakening the overall workflow. Control context before optimizing prompts Over-retrieval is particularly expensive in enterprise systems because source documents are often long, duplicated, and inconsistently structured. Effective context engineering asks three questions: What information is required to complete this step? What can be retrieved only when needed? What can be represented once and reused? Prompt caching can reduce the cost of repeated static instructions, policies, schemas, and reference material, but should support disciplined context design. Stop paying for avoidable agent loops An agent that repeatedly critiques, revises, searches, and retries may produce a better answer, or it may spend most of its budget refining a result that was already acceptable. Enterprises should define explicit limits for: The number of reasoning or refinement cycles Tool-call retries Maximum context size

From tasks to decisions A truly autonomous claims system begins by redefining the workflow. It is not a sequence of tasks. It is a sequence of decisions . Did this loss occur? Is it covered under this policy? Does it fall within acceptable risk thresholds? Should it be paid, escalated, or denied—and why? Humans are excellent at making these judgments in isolation. Machines are excellent at making them consistently, at scale, when the context is complete and the rules are explicit. That qualifier matters. Agents do not fail because they reason poorly. They fail because they reason without context, act without authority, or operate without governance. What production-grade claims automation looks like In a production-grade workflow, teams design autonomy end-to-end rather than bolting it on after intake. The claim arrives . A customer, partner, or system submits a claim through a form, email, mobile app, or third-party feed. The agent ingests the full claim packet . The agent processes structured fields alongside unstructured evidence such as photos, narratives, estimates, and prior correspondence. The agent resolves policy context . The agent identifies the correct policy version, coverage limits, endorsements, exclusions, and applicable jurisdictional rules. The agent attaches historical signals . The agent incorporates prior claims, customer tenure, interaction history, and fraud indicators into the decision context. The agent evaluates coverage . The agent determines whether the loss qualifies for coverage and under what conditions. The agent calculates a settlement . Even the most advanced Large language models (LLMs) face the fundamental limitation of not inherently knowing an organization's current data, processes, or business context. Retrieval-Augmented Generation (RAG) addressed this challenge by connecting AI models to internal enterprise knowledge. RAG allows AI systems to retrieve relevant information from proprietary company data and applications before generating a response. However, as enterprises move toward AI agents capable of completing real business tasks on their own, traditional RAG needs an upgrade. Agentic RAG can handle its own workflows within the context of a business, its data, and its processes. This is the next stage in the evolution of real-world AI applications from a system that simply retrieves information to one that can reason across business data and complete complex workflows. How traditional RAG took AI forward Traditional RAG combines information retrieval with generative AI.
